Field Trips

Field Trip Authorization Form

No student will be permitted on a field trip unless a completed and signed Field Trip Authorization form is submitted to the supervising teacher, sponsor, or school front office at least 48 hours prior to the field trip. Verbal authorization, or authorizations not on this form, will not be accepted.
