Payroll Tax Forms
All forms are due by the 10th of the month and will take effect on the next available payroll.
Payroll Direct Deposit Information
Payroll checks for the Variable and End of the Month payrolls may be directly deposited to either your checking or savings account. This includes Substitute and Noon Duty staff that receive payroll checks on the 10th of the month.
Please submit the following form to start direct deposit by the 10th of the month. The first payroll following receipt of your form will be a waiting period and you will receive a real payroll check.
If you need to change your direct deposit, please fill out a new form by the 10th and keep your old bank account open until you receive the real payroll check during the new waiting period.
If you wish to cancel your direct deposit, please complete and submit the cancellation form by the 10th and the cancellation will take effect on the next available payroll.
